Introducción: El objetivo de este trabajo es valorar la utilidad del estudio del complejo de células ganglionares de la mácula mediante tomografía de coherencia óptica (OCT) para estimar la progresión del glaucoma según su severidad.Material y métodosSe trata de un estudio transversal retrospectivo. Incluye 205 ojos de 131 pacientes con glaucoma o hipertensión ocular seguidos durante una media de 5,7años. Se han analizado los parámetros y las tasas de tres pruebas mediante el software de progresión de cada instrumento: campo visual, OCT en el complejo de células ganglionares de la mácula y en la capa de fibras nerviosas del nervio óptico. Se han evaluado los resultados de cada prueba, la concordancia entre ellas y cómo difieren según el estadio de gravedad.ResultadosEl campo visual clasifica más casos de progresión en el glaucoma moderado-avanzado, mientras que en el glaucoma leve su capacidad está limitada. El OCT de capa de fibras nerviosas del nervio óptico clasifica más casos de progresión en el glaucoma leve que en el moderado-avanzado, ya que se ve artefactado por el efecto suelo. El OCT del complejo de células ganglionares de la mácula es la prueba que más casos clasifica de progresión y que tiene mayor acuerdo con el campo visual, independientemente de la severidad.ConclusiónEl estudio del complejo de células ganglionares de la mácula mediante OCT podría ser mejor biomarcador de progresión que el estudio de la capa de fibras del nervio óptico, en cualquier estadio de glaucoma. Propósito Conocer las cirugías de glaucoma más frecuentes entre los miembros de la Sociedad Española de Glaucoma (SEG).MétodosUna encuesta anónima de 10 preguntas fue enviada a los socios de la SEG en enero, febrero y julio de 2019 para conocer sus preferencias en las cirugías de glaucoma.Variables a estudiar Edad, tipo de glaucoma, tipo de cirugía realizada, estrategia antifibrótica usada o bien implante de las últimas 10 cirugías realizadas desde la recepción de la encuesta. La experiencia del cirujano se muestra según los años de práctica.Resultados Un total de 97 oftalmólogos repartidos por toda la geografía del país respondieron la encuesta, teniendo el 63,4% más de 10 años de experiencia. El glaucoma más frecuente intervenido fue el glaucoma primario de ángulo abierto, representando un 60,6% del total. La cirugía más realizada fue la esclerectomía profunda no perforante (EPNP) con el 37,3% de las cirugías, seguida por el 17,6% de trabeculectomía, 14,1% del microshunt de colágeno (XEN® Allergan Inc., Irvine, CA) y 10,5% de dispositivos de drenaje. Un 21,7% del total fueron reintervenciones y de estas, en el 27,3% se utilizó un dispositivo de drenaje y en el 20,3% trabeculectomía. Un 47,3% de las cirugías fueron combinadas con facoemulsificación. En un 54,8% se usó únicamente mitomicina C (MMC), en un 8,2% de los casos se usó únicamente matriz de colágeno (Ologen®, Aeon Astron Corporation, Taipei, Taiwán) y en un 13,7% la combinación de MMC y matriz de colágeno. La administración de MMC aplicada en esponja se realizó en el 79% de los casos (de estos, a concentraciones de 0,02% en el 99% y al 0,04% en el 1%) y en el 21% de los casos restante mediante inyección subconjuntival (al 0,01% en el 81% y al 0,02% en el 19% restante).Conclusiones A pesar de que el cirujano de glaucoma domina una gran variedad de técnicas quirúrgicas, la EPNP sigue siendo la técnica quirúrgica más realizada en nuestro país (AU)

Mitomycin C (MMC) was used in 54.8% of the cases, collagen matrix (Ologen®, Aeon Astron Corporation, Taipei, Taiwan) was used alone in 8.2% of the cases and in 13.7% combined with MMC. CONCLUSIONS: Although the glaucoma surgeon performs a wide range of surgical techniques, deep sclerectomy remains the most widely used surgical technique in Spain. Combined cataract and glaucoma surgery is performed almost in half of the patients and MMC is the most frequently selected antifibrotic agent, alone or combined with collagen matrix. The new minimal invasive surgical techniques represent the 20% of the total.

Objetivos: El aumento de la calidad y de la esperanza de vida y el hecho de que el glaucoma se diagnostique cada vez más precozmente hace preguntarse a muchos pacientes si afectan a su enfermedad determinados deportes, actividades o aficiones. El objetivo de este trabajo es establecer unas guías para aconsejar a los pacientes con base en la evidencia científica de los trabajos publicados. Métodos: Revisión de todos los trabajos publicados sobre glaucoma y deportes u otras actividades. Los trabajos fueron clasificados según el nivel de evidencia científica basada en la clasificación del Oxford Centre for Evidence-Based Medicine. Resultados: Las actividades aeróbicas son beneficiosas para el paciente. Se han de evitar deportes o técnicas de relajación tipo yoga con maniobras de Valsalva o colocación de la cabeza en posición inferior. También se deben evitar los cambios bruscos en altura. El calor intenso no parece influir en el glaucoma, pero el frío intenso puede afectar a pacientes con disregulación vascular. Las actividades de visión próxima disminuyen levemente la presión intraocular. El uso de instrumentos de viento puede elevar la presión intraocular dependiendo de la técnica utilizada. Conclusiones: Ciertos deportes y actividades pueden tener una influencia en la aparición o la progresión del glaucoma. Los especialistas en glaucoma deben tener información adecuada sobre la evidencia científica de las publicaciones para poder aconsejar apropiadamente a los pacientes (AU)

Activities using the near vision slightly reduce the intraocular pressure. The use of wind instruments may raise intraocular pressure, depending on the technique used. Conclusions: Certain sports and activities may have an influence on the onset or progression of glaucoma. Glaucoma specialists should have adequate information about the scientific evidence in the publications, in order to properly advise the patients (AU)

Objetivos: Establecer guías para aconsejar a los pacientes respecto a la relación entre dieta y hábitos de vida y glaucoma. Métodos: Revisión de los trabajos publicados sobre glaucoma y la dieta, la ingesta de suplementos, el consumo de drogas o el tabaco, el sueño, el embarazo y la hipertensión. Los trabajos fueron clasificados según el Oxford Center for Evidence-Based Medicine. Resultados: La evidencia sobre la relación entre la dieta o el uso de suplementos y la incidencia o la progresión del glaucoma es insuficiente para justificar una recomendación. Si bien algunos estudios en glaucoma de tensión normal sugieren que el Ginkgo biloba podría reducir la progresión, no permiten recomendar su uso generalizado. Los estudios sobre tabaco no demuestran claramente una relación entre consumo de tabaco e incidencia del glaucoma. La marihuana no es un tratamiento útil para el glaucoma. Los resultados sobre la relación entre la apnea del sueño y glaucoma son heterogéneos, pero sí parece conveniente que los pacientes con grado moderado o severo de apnea sean evaluados para descartar glaucoma. El embarazo no suele afectar al curso de la enfermedad, pero varios medicamentos hipotensores pueden ser nocivos para el feto. La hipotensión nocturna es un factor de riesgo de empeoramiento del glaucoma. Conclusiones: Ciertas costumbres, circunstancias o enfermedades pueden tener una influencia en la aparición o progresión del glaucoma. Es importante conocer la evidencia científica existente para poder aconsejar adecuadamente a los pacientes (AU)

Although the results on the relationship between sleep apnoea and glaucoma are heterogeneous, it is recommended that patients with moderate to intense apnoea are tested for glaucoma. Pregnancy does not influence the course of the disease, but several hypotensive drugs may be harmful for the foetus. Nocturnal systemic hypotension is a risk factor for glaucoma progression. Conclusions: Certain habits, circumstances, or diseases may have an influence on the onset or progression of glaucoma. It is important to have adequate information about the scientific evidence in the publications in order to properly advise patients (AU)

PurposePrevious studies have suggested that pregnancy may induce myopia progression. However, no longitudinal study with a large sample size and long-term follow-up has assessed this association. Our objective was to investigate whether pregnancy was related to mid- or long-term myopic refraction changes.Patients and methodsA prospective study was conducted in a Mediterranean cohort. The study included 10 401 women (20-50 years old) from the SUN Project. SUN project is a multipurpose, prospective, and dynamic cohort of university graduates conducted in Spain. The recruitment of participants started in 1999 and it is permanently open. All participants in this cohort had university studies. Participants were followed up for a period of up to 14 years, and pregnancy and refractive changes were assessed through baseline and biennial questionnaires. Pregnancies and myopia were repeatedly assessed in each biennial follow-up questionnaire during a total of 14 years of follow-up.ResultsPregnancy was inversely associated with the risk of myopia development or progression during each of the 2 years periods, with fully adjusted hazard ratio=0.61; (95% confidence interval=0.49-0.75) after adjusting for known potential confounders.ConclusionTo our knowledge this is the first large-longitudinal assessment in young adult women, showing that pregnancy is inversely associated with myopia development or progression. Further studies are needed to confirm these epidemiological findings.

OBJETIVO: Realización de una guía de práctica clínica actualizada sobre el tratamiento médico, láser y quirúrgico del glaucoma por cierre angular primario (GCAP) en el adulto. MÉTODOS: Tras la formulación de preguntas clave utilizando el esquema PICO (Paciente/Problema, Intervención, Comparación, Outcome/Resultado), se realiza una revisión de la literatura publicada hasta el momento, incluyendo guías de práctica clínica internacionales, utilizándose las herramientas AMSTAR y «Risk of Bias» para la evaluación de la calidad de la información. El nivel de evidencia y la elaboración del grado de recomendación se establecieron siguiendo el sistema Scottish Intercollegiate Guidelines Network (SIGN). RESULTADOS: Siguiendo la metodología expuesta, se presentan recomendaciones de tratamiento médico, láser y quirúrgico en el GCAP del adulto y los niveles de evidencia. CONCLUSIONES: Aunque el nivel de evidencia científica para muchas de las preguntas planteadas no es muy alto, se presenta una revisión actualizada de las recomendaciones terapéuticas en el GCAP del adulto. Entre las limitaciones para la aplicación de estas recomendaciones se encuentra que la mayoría de los estudios han sido realizados en población de origen asiático, y que la eficacia se mide casi exclusivamente en términos de reducción de tensión ocular y no en parámetros de función visual, calidad de vida o coste-efectividad


OBJECTIVE: To present a clinical practice guideline update on the medical, laser, and surgical treatment of primary angle closure glaucoma (PACG) in adults. METHODS: Following the formulation of key questions using the PICO scheme (Patient/Problem, Intervention, Comparison, Outcome), a systematic review was performed on the literature published to date, including international clinical practice guidelines. The AMSTAR and Risk of Bias tools were used for evaluating the quality of the information. The level of evidence and grade of recommendation was established following the Scottish Intercollegiate Guidelines Network (SIGN) system. RESULTS: Following the above methodology, recommendations of medical, laser and surgical treatment in adult PACG and levels of evidence are presented. CONCLUSIONS: Although the level of scientific evidence for many of the questions raised is not very high, a review is presented on updated treatment recommendations for adult PACG. Among the limitations for the implementation of these recommendations is that most studies have been conducted in Asian populations, and that the effectiveness is measured almost exclusively in terms of reducing intraocular pressure, and does not include visual function, quality of life or cost-effectiveness parameters

OBJECTIVE: To present a clinical practice guideline update on the medical, laser, and surgical treatment of primary angle closure glaucoma (PACG) in adults. METHODS: Following the formulation of key questions using the PICO scheme (Patient/Problem, Intervention, Comparison, Outcome), a systematic review was performed on the literature published to date, including international clinical practice guidelines. The AMSTAR and Risk of Bias tools were used for evaluating the quality of the information. The level of evidence and grade of recommendation was established following the Scottish Intercollegiate Guidelines Network (SIGN) system. RESULTS: Following the above methodology, recommendations of medical, laser and surgical treatment in adult PACG and levels of evidence are presented. CONCLUSIONS: Although the level of scientific evidence for many of the questions raised is not very high, a review is presented on updated treatment recommendations for adult PACG. Among the limitations for the implementation of these recommendations is that most studies have been conducted in Asian populations, and that the effectiveness is measured almost exclusively in terms of reducing intraocular pressure, and does not include visual function, quality of life or cost-effectiveness parameters.
